Eligibility Screening

Clinica Adelante, lnc. is dedicated to providing a full range of quality primary care services to all residents regardless of their ability to pay. Patients without health insurance may qualify for programs and services through reduced fees based on income and family size. To apply for affordable services in any of our programs please bring the following to your first appointment:

• Proof of income (paycheck stub, most recent IRS Tax Return, or letter from an Employer)
• Proof of address (utility bill, rent statement, AZ motor vehicle registration)
• Social Security Numbers for all family members who have them (if applicable)
• Names and relationships of all household members

Eligibility determination for primary care may also include reduced fees for additional secondary services for Pharmaceuticals, Laboratory and Radiology.
