Appointments

Patients who have scheduled appointments will be given priority. Many appointments are available later the same day or within 1-2 days. If you must cancel or reschedule, please provide us with at least 24 hours notice. Walk-in patients may be seen based upon availability.

What to Bring With You

When you come to your appointment please plan to arrive 15-20 minutes before your scheduled appointment time to update your medical records. We will need your current address, telephone number, insurance card, proof of income, and family size. Also please bring your medications and immunization records.

After Hours Care

Dial 911 for life  threatening emergencies. For urgent medical problems after hours, call 623-546-2294 to leave a message for the doctor on call.

Prescriptions

For refills, please call your pharmacy at least three days before your medication runs out.

Bring your medications to every appointment so that we can review them for any changes or possible drug interactions.

We are unable to schedule appointments or refill medications after hours.
